Rob - that's been my experience with oral cannabinoids (dronabinol in my case) as well - limited effectiveness. On the other hand I have many patients who genuinely feel that they greatly benefit from smoked marijuana w/r/t appetite, nausea, and general well-being. I'm rather agnostic about the whole thing....

Alex - thanks for referencing my post and for the links. Your war on pain blog is quite a read and I'd recommend it to our readers interested in pain policy.

The link I posted in my previous Comment is annoying - it gets you to the correct page allright, but places you at the end of the entry at the "Footnotes" section at the bottom of the post, which is disorienting.

Not sativex...I have used oral cannabinoids for the odd case of refractory nausea...can't say I've noticed any profound benefit in those few instances...maybe I just haven't used them enough, but somehow I doubt it...

Hi Drew...have to say I've been underwhelmed by the data on Sativex and have rushed to use it even though it has been available here for some time now. The main issue being that the only trials I've seen, like this one, focus on pure neuropathic pain states...a rarity in our buisness. Until I see evidence of efficacy in cancer patients with mixed pain states, I'm keeping my prescription pad in my pocket...Rob
